POSTSTERNOTOMİ VE ÖZEFAGUS PERFORASYONLARINA BAĞLI MEDİASTİNİTLER

Background:

Mediastinals is a severe complication of coronary artery bypass surgery and esophageal perforation. We retrospectively determined the preoperative and intraoperative risk factors for mediastinitis, and the impact of this complication on mortality.

Materials and Methods:

A retrospective chart review was performed of all patients treated by the Department of Thoracic and Cardiovascular Surgery from 1998 to 2002. The organisms responsible for the infection, leucocyte counts, chest X-ray findings, and treatments were recorded.

Results:

Records of thirty-three patients (21 male; age range 5-79 years) were reviewed. Most important risk factors were the length of bypass and the lateness in diagnosis of esophageal perforation. The most common isolate was methicillin-resistant Staph, aureus. Common chest X-ray findings included pneumonia and pneumothorax. Patients ware managed operatively and non-operatively.

Conclusion:

Rapid diagnosis and treatment of mediastinitis, with adequate supportive therapy, can result in a low mortality rate among mediastinitis patients.
