Acute urinary retention is a urologic emergency characterized by the sudden inability to urinate. The urgent treatment of acute urinary retention is decompression of the urinary bladder using urethral or suprapubic catheter. Acute urinary retention includes obstructive, neurogenic , pharmacologic and extraurinary causes. Rectal foreign body is uncommon cause of acute urinary retention and especially seen in young patients.

In this case a 42-year-old male patient applied to emergency room with a complaint of anuria due to rectal foreign body as a result of autoerotic manipulation.

Rectal foreign body had led to complete urethral obstruction with a mechanism similar to posterior urethral valve . The surgical extraction of the rectal foreign body had led to improvement of the infravesical obstruction symptoms completely and quickly.

This case was presented to emphasize the importance of a complete physical examination including the rectal examination besides sufficient medical history in young patients who diagnosed as acute urinary retention in emergency room.
