Impact of NEXUS Low-Risk Criterias on cervical injuries in Turkish tertiary care hospital

Aim: In our study, we aimed to determine the amount of unnecessary X-ray graphics that could lead a cost-load and radiation side-effects by using NEXUS Low-Risk Criteria that was applied and evaluated for patients with cervical spine trauma in Izmir Ataturk Training and Research Hospital’s emergency department.

Materials and Methods: Between June 2010 and Setember 2010, the trauma patients admitted to emergency service, were evaluated retrospectively. The age, sex, NEXUS Low-Risk Criteria, mechanisms of trauma, radiological imagings, cost,radiation load and degree of cervical injuries were assesed.

Results: In the study, 1317 patients with a possible cervical spine injury were included. It was confirmed that the number of excluded and included patients according to NEXUS Low-Risk Criteria was 1014 and 303. In excluded patients, out of 479 (36.3%) did not have any radiology but in 535 (52.7) of them were administered unnecessary radiological imaging studies. The trauma mechanism was crash at the same level in 310 (23.5%) of total and 267 (26.3%) of excluded patients. Also, trafic accident (patient in vehicle) was found in included patients of 64 (21.1%). It was found that the number of cervical spine injury was 4 in the excluded and 10 in included groups that were confirmed according to NEXUS Low-Risk Criteria.

Conclusion: The increase in the usage of NEXUS Low-Risk Criteria lead a decrease in both of the cost and the amount of radiation exposure in patients and health workers. Also, the number of cancers due to iatrogenic etiologies in human will get lower.
